Dr Amit Singh - GP Partner - Vrach 1995 People's Friendship University Moscow

Dr Singh is the clinical lead for the service in Conwy having joined the partnership in 2021 from Penrhyn Bay Surgery.  He has a passion for developing a patient centred service.  

 

Dr Chris Dunn-Jones - Salaried GP - MB ChB 1988 University of Manchester

Full registration date:  August 1989         Gender: Male

Dr Dunn-Jones joined the surgeries in February 2022 having worked for 27 years in primary care in York.  He has an interest in Cardiology, Minor surgery, Diabetes & Medical education.

 

Dr Corina Anders- Salaried GP-

Cornia joined the pratice in  April 2024. Her interests are in paediatrics and gynocology.

 

 

Clinical Support Team

 

 

Sarah  - Advanced Nurse Practitioner

Registered Nurse: March 2004          Nurse Prescriber: July 2017

Sarah is our Advanced Nurse Practitioner and our Lead Nurse. Sarah is able to triage, assess, diagnose and treat patients. She is able to prescribe medication, and is committed to the nursing provision at Conwy, whilst also supporting the activities of the GPs.

 

Jacqui- Advanced Nurse Practitioner

Jacqui joinned us in June 2024. She is able to triage, assess, diagnose and treat patients and able to prescribe. Jacqui is with us 1 daya a week, speanding the rest of her time in Plas Menai.

 

Sarah  - Advanced Physiotherapist

Sarah joined the Plas Menai health centre in 2021 and now splits her time between Plas Menai and Conwy as our Advanced Physiotherapist.  She leads on our Musculo-skeletal and physiotherapy needs and is available across for 1 1/2 days in Meddygfa Gyffin.

 

Josephine  - Community Clinical Practitioner       

Josie joined the surgeries in 2022 from the local Community Team. Josie supports the clinical team by offering follow-up home visits and supporting newly discharged patients who are housebound. She is the lead for Learning Disability care reviews. Josie also supports the Practice Nursing Team providing dressing clinics and Diabetes care. 

 

Stephanie - Pharmacy Technician

Stephanie is our Pharmacy Technician and has been employed at the surgeries since April 2022.

The pharmacy technician role includes processing medication queries from patients and staff, advising on sourcing and safe management of medicines as appropriate, assisting in submission of claims for certain medication, interacting with patients by participating in medicine reviews and liaising with local pharmacies and healthcare providers.

 

Leah - Occupational Therapist [Local Health Board]

Leah is an Occupational Therapist employed by the health board.  She has been offering an Occupational Therapy service within Meddygfa Gyffin and Llys Meddyg surgeries since 2022. 

Occupational therapists enable people living with a range of health problems and chronic conditions to overcome barriers so they can participate in everyday life, and improve their health and wellbeing.

 

Nursing Team

Gwen  - Practice Nurse

Gwen is our practice nurse. Gwen oversees the daily running of the treatment room. She also runs the diabetes clinics and travel assessments.

 

Julie and Trish- Health Care Assistants

Julie and Trish spend 1 day each at Meddygfa Gyffin. They provide initial diabetic screening and offer a wide range of injections (B12, shingles etc).

 

Su- Phlebotomist

Su joined the team in November 2023. She provides phlebotomy services and Blood Pressure monitoring at both Meddygfa Gyffin (3 days a week) and Llys Meddyg (twice a week).

 

Management Team

 Sarah Bates - Practice Manager

 Responsible for the safe running of the day to day activity at Meddygfa Gyffin and Llys Meddyg Surgeries.

 Amanda- Operations Manager

 Supports Sarah and manages the Care Navigation Team
