Register as a New Patient

The Llanfairfechan Group Practices Partnership is responsible for delivering primary care services to:

To register as a patient of any of the above practices, we ask that you either:

  1. Attend your chosen health centre/surgery and complete the necessary GMS1 form to register - you will need to present your medical card and photographic identity to reception who will then arrange your registration. 
  2. Complete the online registration document below - this will support your registration however you will still be required to then attend the surgery to prove identity and your address.

Finally you will need to complete the new patient questionnaire which is below.

Once you are registered, we offer new patient health check appointments with our practice nurses. If you wish to book one, please call the relevant surgery on the above contact numbers.

Registered patients aged over 75 years who have not been seen in the previous 12 months by a GP may request a consultation. If you are unable to attend the surgery for this consultation because of your condition, a home visit may be arranged.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with the practice.

Please note that practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.

You cannot register as a temporary patient at a practice in the town or area where you are already registered.
